I’m ready to enroll

Go to nationwide.com/enroll and select “Enroll now” or fill out the following forms. To get started with online enrollment, you will need your plan case number 319-86503.

Gather or locate the following information you may need

• Yourpersonal contact information
  • Your annual income and pay frequency
    • Your Plan’s case number (listed above)
      • Yourage when you plan to retire
      • Yourbeneficiary information

Go to nationwide.com/enroll

• Click on Enroll now
• Enter your Case number

• Enter your Social Security number

• Enter your Date of birth
• Click Get started

Click Go when your employer’s name appears. During your enrollment, you will be asked to:

• Enter the rest of your personal information
• Determine how much you will contribute out of each paycheck
• Choose which investment(s) you will invest in
• Enter your beneficiary information
• Confirm and submit your enrollment
